Please note that the Jane App Intake form is different from the Patient Registration form.

All patients MUST fill out the Patient Registration form and Email and Scribe consent before booking an appointment.

New Patient Procedure

We aim to make your transition into our care as seamless as possible. Follow these simple steps to begin your health journey with HIFY.

1. Register as a New Patient

The first step is to establish your profile in our system. Visit the link below to fill out the form.

2. Book Your Appointment

Once registered, you can schedule your consultation. Choose a convenient time via our online booking system by clicking the "Patient Portal" in the top right corner. This appointment must be a "Meet and Greet".

3. Complete Digital Pre-Arrival Forms

Save time in the waiting room by completing your digital questionnaires and consent forms online. Please see below:

4. Attend Your First Visit

On the day of your appointment, please bring your BC Services Card (CareCard) and a list of any current medications. We look forward to meeting you and discussing your health goals.

5. Follow-Up & Ongoing Care

After your initial consultation, we will coordinate any necessary follow-ups. You can manage your ongoing care plan and future appointments directly through your Patient Portal.

First Visit FAQs

What should I bring to my first visit?

Please bring your BC Services Card (CareCard), a piece of photo ID, and a list of any current medications or supplements you are taking.

What are the technology requirements for telemedicine?

You will need a device with a camera and microphone (smartphone, tablet, or computer) and a stable internet connection. We use a secure, encrypted platform accessible via your web browser.

Do you accept my insurance?

Most clinical services are covered by MSP for BC residents. For non-insured services or private insurance questions, please contact our office directly.

How do I cancel or reschedule an appointment?

Cancellations can be made through the Patient Portal or by calling our office. We request at least 24 hours' notice to avoid potential cancellation fees.

Is the Patient Portal secure?

Yes. Our portal uses industry-standard encryption and security protocols to ensure your health information and communications remain private and compliant with BC privacy regulations.
