Third-Party Physician Form Request

Note to Patients: Third-party forms are not covered by MSP. They are billed privately in accordance with the Doctors of BC uninsured services fee guide and our clinic policy. Fees are due before completion of the form.

Purpose of request:

I authorise my physician and clinic staff to complete the requested third party form, including disclosure of relevant medical information as required by the form.


I understand that MSP does not cover these services and must be paid out-of-pocket.


Fees are non-refundable once the form is processed.


Additional charges may apply for lengthy or complex reports.


If the requesting agency returns the form due to a clerical or factual error made by the clinic, it will be corrected and resubmitted at no additional charge.
